Position Summary: 

 

The RN Team Lead (RNTL) is the lead escalation point for clinical issue resolution and management of emerging clinical issues. The RNTL works with care teams to manage risk, track and respond to changes, and identify the need for at-risk case management processes. The RNTL works with care teams to ensure compliance with documentation standards, required regulations, and best practices. The RNTL carries a half-time RN Care Manager caseload.

Responsibilities

 

  • Responds to requests from caregiver and consumers and communicates with team members regarding concerns or emerging issues.
  • Tracks consumer risk and works with the care team to adjust the plan of care (POC) as appropriate.
  • Assesses the need for at risk case conferencing and oversees the team process and follow-up for at risk case conferences.
  • Conducts home visits with clinical team members as needed.
  • Performs quality reviews of consumer and caregiver records on a quarterly basis.
  • Identifies individual or team training needs and provides training to new clinical team members and others as needed
  • Maintains working knowledge of computer and hand-held devices and systems, including case management systems, community service and delivery systems, internet applications, and the Microsoft Office suite.
  • Performs registered nurse care manager job responsibilities as required.

Qualifications

Required Education / Experience


Bachelor’s degree or equivalent experience preferably in a health care or social services discipline required. Minimum of three (3) years of clinical experience in case management and program operations in the areas of healthcare and social services, including but not limited to eldercare, people with disabilities and other complex medical needs. Additional state-specific qualifications may be required. Valid driver's license required.
