Position Summary:

 

The Community Health Worker works collaboratively with the Care Team to implement a person-centered plan of care as well as conduct regularly scheduled home visits. 

What You Will Do:

  • Assist Care Manager and/or Family Specialist with home assessments, collection of required caregiver qualification documents and ensuring that all required home modifications are completed.
  • Deliver consumer-specific training to caregivers on an ongoing basis and document training and caregiver competency.
  • Complete daily review of case record documentation for accuracy, completeness, compliance, and timeliness and conduct follow-up as needed.
  • Maintain working knowledge of computer and hand-held devices and systems, including case management systems, community service and delivery systems, internet applications, and the Microsoft Office suites.

What You Will Bring:

  • Bachelor’s degree in Social Work or high school graduate with 3+ years of experience working within the community health setting in a healthcare role.
  • Community Health Worker (CHW) Accreditation
  • Bilingual preferred
  • Must have resided within the local community for 2+ years.
  • Familiarity with the resources available within the community.
  • Must have a valid driver’s license
